The Eye Center of Southern Indiana is dedicated to providing you the best eye care with the attention you deserve. Our mission is to enrich your quality of life through technologically innovative methods of enhancing images. The following information will help guide you through the process and make you aware of what to expect on your visit with us.
To schedule an appointment with us, you may call (800) 222-7576 or (812) 334-1213 and we will direct you to the appropriate time and physician. Or you may complete the information below and a staff member will contact you as soon as possible.
Prior to Your Appointment:
Please complete and print the patient registration form and medical history form even if you are an established patient. Once you have completed the forms, please bring them with you on the day of your appointment. Due to HIPAA privacy concerns, please do not attempt to send these forms to our office via e-mail.
Please check with your insurance company(s) to verify your medical coverage for routine eye examinations by calling the customer service phone number listed on your insurance card.
If you have a medical clinic listed on your insurance card, you will need to call its referral department to determine if an insurance referral is required for coverage of your eye examination, medical visit, or procedure. If you need further information to process this referral please call (812) 334-1213 and ask to speak to our insurance coordinator.
Should you need financial assistance for your services, please call our office before your visit to find out what options are available to you. The insurance and billing office is available Monday - Friday from 8:00 a.m. until 5:00 p.m. at (812)353-6467.
The Day of Your Appointment
At the time of your visit, you will be asked to complete a Patient Registration Form and a Health History Form. If you have already printed and completed these forms at home, please be sure to bring them with you.
We will ask to make copies of your insurance card(s).
Please make sure you bring the most current insurance cards with you, as insurance companies frequently modify benefits. If your insurance, address or phone number changes at any time throughout the course of your care with us, please notify us promptly.
The co-pay amount listed on your insurance card will be collected at the time of your visit, if applicable.
You will be responsible for any co-pays and deductibles, as well as any services not covered by your insurance policy. For charges not covered by your individual insurance plan, we require payment in full at the time services are rendered. Prompt pay discounts are available on certain services. Please ask at the front desk. We accept cash, check or Visa, MasterCard, Discover, or American Express as payment for services.
You may want to bring sunglasses with you to your appointment.
As part of your examination, your pupils may be dilated. With dilation, most people experience light sensitivity and difficulty with close focusing (such as reading). Some individuals also have difficulty seeing in the distance. The effects of the dilation can last up to several hours. Please ask any staff member if you should have any questions or concerns about dilation.
