Billing

The physicians at The Eye Center of Southern Indiana are contracted providers for many insurance companies, including traditional Medicare, most Anthem BC/BS, and Indiana Medicaid.

We will file medical insurance claims on your behalf regardless of our network participation status, with some exceptions:

• We must be able to confirm the patients identity, using photo identification.
• We may not file a claim for services that are considered cosmetic or not medically necessary.
• We will inform you in advance if a procedure is considered cosmetic or not medically necessary.  All cosmetic procedures are to be paid in full at the time of service.
• If you have questions about your insurance coverage at this facility, please request a financial consultation prior to the start of your examination.  Ultimately, you are responsible for understanding what your insurance carrier pays for and what providers are in your network.

Patients who live out-of-state or who have no health insurance coverage are asked to pay for services as they are provided.

Co-payments are expected at the time of service.
