Getting Ready

In an effort to make your visit as comfortable as possible, we have created this check list to help you prepare for your visit. The items marked with an asterisk are mandatory items that you must bring with you. The additional items are forms that you can fill out at our offices. You may find it more convenient to download these forms and fill them out at home prior to your visit.

Office hours are 8am-5pm, Monday through Friday. We are closed for lunch from 12pm to 1pm every day.

You will need a username and password to access the forms. If you have not already been given these you can call our offices at 812.334.1213 or 800.222.7576. Usernames and Passwords are only available during normal office hours. Our normal office hours are posted here.

Checklist

1) A state issued photo ID
2) Your insurance card (if you have one)
